1. Please give detail on the profile of your organisation. This can include its history, operations, staff numbers, range of products and services and locations. Please add any other information you feel is appropriate.
2. This award is for the business which demonstrates success in international markets. By international we mean outside the UK and Republic of Ireland. Please detail the products and services offered by your organisation and identify the international markets where they enjoy success.
3. Please outline the drivers behind your move into international markets and identify two challenges you faced with this move and how you met these challenges.
4. Please outline your business plans for expansion over the next five years. Please include details of growth in revenue, operating margin, staff number(s) and outline any new products or services you plan to introduce.

The application for the paper based categories may be made via email or post and should consist of 4 sides of A4 paper and not to exceed 2000 words. For hard copy posted applications, 4 x copies are required.
Business activity within the last 3 years means from January 2007 - Januray 2010. Business activity within the last 5 years means from January 2005 - Januray 2010.
Additional supporting items such as brochures, CD ROMS, etc will only be considered as additional information to the required entry material and supporting items cannot be returned. For Mystery Shopping Awards complete the application form only either online or by post. All entries must be made using the official entry form or its online counterpart.
