1.       Please provide an outline of the profile of your enterprise.

          This can include its history, operations, staff numbers, range of products and services, key beneficiaries and locations. Please add any other information you feel is appropriate.

 

 

2.       Please outline how your enterprise contributes to the social economy sector. As well as details of your project, we would also like you to outline the level of re-investment into your enterprise and the community as well as the added social value your enterprise contributes.

 

 

3.       Please outline two challenges your enterprise has faced over the last three years and what change in strategy was undertaken to meet these challenges.

Entries are available in 19 categories. Organisations may enter as many of the award categories as they wish. 

Entries are open to all companies, organisations, public sector providers and not for profit organisations with the following restrictions: entries must be from businesses that are located in Belfast City Council area.

The  application for the paper based categories may be made via email or post and should consist of 4 sides of A4 paper and not to exceed 2000 words. For hard copy posted applications, 4 x copies are required.

Business activity within the last 3 years means from January 2007 - January 2010.  Business activity within the last 5 years means from January 2005 - January 2010.

Additional supporting items such as brochures, CD ROMS, etc will only be considered as additional information to the required entry material and supporting items cannot be returned. For Mystery Shopping Awards complete the application form only either online or by post. All entries must be made using the official entry form or its online counterpart.
